Duties

• Diagnose and manage acute and chronic health conditions across diverse populations.
• Develop and implement individualized care plans based on patient needs and evidence-based practices.
• Collaborate with interdisciplinary teams in nursing homes, memory care facilities, and emergency medicine settings to deliver coordinated care.
• Educate patients and their families on health maintenance, disease prevention, and management of chronic conditions.
• Maintain accurate patient records in compliance with healthcare standards and regulations.

Skills

• Familiarity with diabetes and thyroid management.
• Excellent communication skills for effective interaction with patients, families, and healthcare team members.
• Strong organizational skills to manage multiple patient cases efficiently while adhering to high standards of care.
